"Were" in this context is the most appropriate. In most cases, when presented with a situation where the opposing person talked about something that you forgot, "were" is better. "Are" can range from being awkward to confrontational. However, if you said this sentence at the start of the conversation, "are" would be much better suited.

Both can be correct, but it depends on some context. Usually, "are" would be correct in most circumstances. If we're meeting for the first time, you would ask me "What state are you from?" However, if you're trying to recall which state I am from or if I have moved to a different state, you could ask me "Which state were you from again?"

Both are actually okay in this cirumstance! "Are" sounds more like you're just asking me where I'm from, whereas "were" sounds like you're trying to remember when I mentioned it before, in the past, but are still asking me the same thing.
