Why are our apologies sometimes not accepted ?

In this world, nobody hasn't ever made a mistake, especially when we grow up and make some social relationships. We say apologise to show that we've filled with remorse for what happened to the victim, ready to take responsibility and have a desire to patch up that relationship.
