Sentences

I haven't been to the theatre since I go to school.

This is technically correct, but without more context, it sounds odd because the cause and effect don't have an obvious relationship. Some alternatives:
"I haven't been to the theatre since I was at school all day."
"I haven't been to the theatre since I've been occupied with my studies."

Obviously, question 3 inof the test is incorrect.

I haven't been to the theatre since I go to school.

I understood this as a sentence about causation and read "I haven't been to the theatre" as a statement about a present state. In that case, it's not impossible to use the present tense with it (though the OP's case is awkward), especially when there is a habitual aspect.
"I haven't been to the theatre [lately], since the performances are on Shabbat."
"I haven't been to the theatre; it doesn't interest my partner."
But I see where your correction is coming from as well.

Sentences

I haven't been to the theatre since I go towas in school.

"go to school" is the present tense. To describe a time in the past, it's better to use "was".

Obviously question 3 ion the test is incorrect.

This sentence looks pretty good, although saying a question was "on" a test feels more natural than saying it was "in" a test.
