He's saying "If that shit's contagious..." but I don't hear "If that" at all. Can native speakers hear it? How can it be represented by a diacritical mark?

I dont think its very common, but sometimes it happens when people talk fast. It may be common for different regions.

You can skip “that” in many noun clauses that follow.
Ex: I hope (that) he speaks properly at the wedding.
However, in certain dialects in some Southern parts of the US, you might hear types of vernacular that are not “proper” English. That could explain why you can’t hear certain words because those dialects skip those words.
