The second version isn't incorrect per se, but it is quite archaic and may make seem quite posh. Or it sounds like it comes from a children's story book. One of the two.

"Never was she on time!" is something you'd only see in literature though. It isn't natural in real speech.

COMMENT: *Both* are possible, but the emphasis is different. In English, the 1st word in any sentence is emphasized more. Hence, "She was never on time." emphasizes the word "She." The sentence "Never was she on time!" emphasizes the word "Never."
