Could you help me with an expression that I can describe about one of my friends.

She is always late.

She forgets schedule all the time, but she doesn't write anything on her schedule notebook or phone.

The other day she told me that she cooked something and forgot to eat it, and a few days later, when she noticed, it had already gone bad.

Can I describe her "She is always not together" or "She doesn't always put together"?

Schedule is general but appointment is more a particular time. If she's a student then maybe classes, or if working adult, meetings. Could also use plans. Guessing by schedule you mean calendar? If she has google, she can use the google calendar (and even share it with others).

This is very slangy (and generally considered rude so you wouldn't say this to the person directly but it seems to fit "She is a hot mess" . You can also say "she doesn't have her house in order".

Or are there any better phrases instead?

This was a fun read! Maybe google calendar is a good option for her. I have a couple of shared calendars that are useful for the two of us. Sometimes if we do a group trip, we will share the calendar with the group. It's pretty nice. But it only works if you put the data into the calendar. If she never does that then it won't really help (¬▂¬)

I think you are saying “schedule, notebook, or phone” as 3 different things. If you mean the schedule in her notebook or phone you can say that instead!
