Hi Jack, the difference between 'can' and 'could' is subtle but very important if you want to sound more like a native speaker. Basically 'could' refers to the past whereas 'can' refers to the present. So when you write 'how can he get this disease' you're speaking about the past so 'could' is more appropriate here. The more you read and write in English the better you will understand this difference. Even as a native speaker I'm finding it difficult to describe how and when to use 'could' rather than 'can'. Here are some example sentences that sound natural: 'how can we be better people?'; 'how could someone do something so cruel?'; 'why can't humans fly?'; 'why couldn't we just be friends?'; 'who can play here?'; 'who could have done this?'.
