Inefficient Meetings

I attended an extremely inefficient meeting which lasted nearly 3 hours this afternoon. In the meeting, people argued about several detailed issues and everyone was very stick to their personal opinions, they didn't listen to each other carefully. The most frequent sentence was "Our boss had said we can't do that". In the end, there was no any conclusion.
After the meeting, I though why the meeting could be so bad. I looked back at several other meetings and found they were similar to the meeting this afternoon, maybe it had been a common phenomenon in our company. At last, I thought the main reasons might be as follow:
1)The staff in our company were too obedient to(even afraid of) their superiors to listen to different opinions with their superiors.
2)The selection of participants was not fit. There was no decision makers.
3)No one stoped the ineffective arguments, so the meeting lasted such a long time.
Hope this situation could be improved in the future.
