It is said that in the past you would just answer with "How do you do?" back as an introduction.
But nowadays, you can just think of it like being asked "How are you?" or "How is it going?"
So the simplest would be to just say "I am fine", or "I am doing great" then follow with "Yourself?"
You can also just mention your state at the moment, "Eh, just so so, it's Monday after all, how about you?"
