A: Yeah. Maybe not, but in Japan, when I say an accident by train, it is usually a suicide.

We'd never use this phrase in the US while speaking to a total stranger: "causing injury or death"

We'd never say what you wrote originally either. It's too direct.

Maybe not, but in Japan, when I say a train accident by train, it i's usually a suicide.

The last half is quite formal.

A: Yeah.
Maybe not, but in Japan, when I say an accident by train, it is, maybe not. In Japan, 'accident' usually ameans suicide.
B: Oh my gosh.
